Undergraduate Scholarship


New OATS Scholarship available for HS seniors in the amount of $200. Deadline for application is May 1st. Winner will be notified by June 1.  Fill out application or for more information, please contact Cuyla Markovich, cdudleyatc@aol.com

Eligibility Requirements

All Applicants Must:

  • Be a high school senior at the date of the application
  • Intend to earn a degree in the medical field
  • Have a minimum 3.0 GPA based on a 4.0 scale
  • Be sponsored by instructor/ATC who is an OATS member
  • Submit an application postmarked on or before May 1, 2004

Scholarships:

The OATS Scholarship. To qualify, the applicant must enroll in a university, community college, or technical college.

Nomination and Selection for the Scholarship:

The OATS Scholarship Selection Committee will consider all nominations in detail and individually. To be considered for this select award, a student must be nominated by a letter or recommendation from an individual who has substantive knowledge of the student’s accomplishments and goals. The selection committee finds it helpful to learn specific examples of leadership, academic, and personal achievements that set this student apart from his or her peers. In addition to outstanding academic performance, the committee is interested accomplishments in the sports medicine field.

Selection Committee:

The Selection Committee will consist of Certified Athletic Trainers who are members of OATS Honors and Awards Committee. These individuals will score each individual applicant, and total scores will then determine the overall winner.

Application Instructions:

1. There are three sections of the Scholarship Application to be completed:
a. An application to be completed by the nominee and signed by the nominee and nominating Sports Medicine Instructor
b. Two letters of recommendations, one of which needs to be by a Certified Athletic Trainer / OATS member.
c. An essay and short answer responses to be written and signed by the nominee.

2. Incomplete applications will not be processed. All applications must include and official transcript, necessary signatures, and other requested information.
3. The nominating OATS instructor is considered to be the individual responsible for collecting all official transcripts and the completed application, and forwarding them to the Scholarship Selection Committee.
4. The scholarship application must be postmarked on or before May 1, 2004 to be eligible for the scholarship.
5. All pages of the application must be typed.

Student Essay:

You must prepare an essay of no more than one page typed. It must be signed and dated. Respond to the following prompt.

Looking ahead ten years, describe the changes In health care that you believe will occur.

Student Narrative:

You must prepare responses to the typed student narrative. It must be signed and dated. Respond to the following statements. One half page each for a total of one page.

Describe your academic, educational achievements, and awards including accomplishments in the sports medicine/medical field.

Explain activities and qualities that demonstrate leadership, citizenship, and interpersonal skills that have provided a positive contribution to your school and /or community.

Notification of Awards:

Recipient of the OATS Award will be notified by June 1 and recognized at the OATS Summer Symposium. (They do not need to be present)
